An authorization to conduct a criminal history check is required of NISD volunteers or contract providers who will work directly with students. The NISD Human Resources department will be responsible for conducting criminal history checks. Please download the enclosed form, sign and fax back to 210.706.8627. The online form is now available and can be submitted here.
